Healthwise
Healthwise is a Physical Activity Referral Scheme (PARS), where Health Professionals can refer patients to a low cost physical activity programme. It is designed for individuals with existing health conditions, such as CHD and Asthma, as well as those at risk of developing health conditions, including CHD, diabetes, obesity and depression.
Healthwise is designed for those patients who are not currently active, but who would benefit from physical activity. We recommend that you explore thoroughly with each patient whether they are ready to start an exercise programme, and ask that you read the inclusion criteria carefully before referring a patient. This is not a long term solution for those requiring one-to-one attention.

Healthy Lifestyles
The Healthy Lifestyles Scheme is an innovative project working with 7-13 year olds in Hackney and the City who are overweight. The programme offers support to young people in achieving healthier lifestyles through physical activity, healthier eating and behaviour change, with an emphasis on having fun and building self-esteem. Participants are offered a structured 10-week programme of sessions covering a wide variety of health-related topics and parents/carers and other family members are also supported and encouraged to participate. GPs, school nurses, teachers and other professionals may refer participants into the scheme and potential clients or their families may also self-refer.

CHYPS Plus
CHYPS Plus offers services for young people aged between 11 and 19 years old at The House behind Lower Clapton Health Centre. They complement existing general medical services offering adolescent services and resources. The environment is confidential, and the team is specially trained in providing youth centred care.

CHYPS Plus offers a holistic and flexible service that is available at times most convenient to young people. A doctor and nurse are on site as well as development workers, counsellors and dieticians. Obesity is one of the five focus areas for the service along with sexual health, chronic disease management, smoking and mental health.
